Care management’s overarching goal is to improve patient health. To get there, the model also aims to improve care coordination, reduce hospital visits and boost patient engagement.
Care Management is a pro-active, patient centered approach designed to help patients and their support systems -- including their family members -- more effectively manage chronic illness. Successful care management is designed to enhance the coordination of patient care, reduce the risk of complications, and help prevent costly hospital admissions and readmissions.

Our care management team can help you connect with needed community resources for securing food, shelter, in-home support and other needed services.

Comprehensive care management demands a team effort. Physicians, clinicians, patients and their caregivers must all work together to help patients take control of their complex health needs.
